A 401-page independent review has exposed years of dangerous maternity and neonatal care at Nottingham University Hospitals NHS Trust. Published on June 24, 2026, it examined experiences at Queen’s Medical Centre and Nottingham City Hospital, mainly between 2012 and 2025.

Led by senior midwife Donna Ockenden, the investigation found that serious clinical mistakes were reinforced by understaffing, bullying, racism, weak leadership and repeated failures to listen. The findings explain how warnings remained unresolved while more families suffered avoidable harm.
